The PHI involved included names, addresses, birthdates, drivers’ license numbers, health insurance information, Social Security numbers, financial information, lab results, medications prescribed, diagnoses/conditions, and treatment information. The CE notified HHS, affected individuals, the media, and local law enforcement. In its mitigation efforts, the CE sanctioned the employee involved and retrained its staff.
