The covered entity (CE), Washington University School of Medicine, reported that it experienced a hacking incident that affected the protected health information (PHI) of 1,056 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses/conditions, and medication names. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards to better protect sensitive data.
