The covered entity (CE), University of Miami, reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 86,000 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notification on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. It also provided complimentary credit monitoring and identity protection and restoration services. The CE subsequently ended its business relationship with the BA.
