The covered entity (CE), Priority Health, reported that a box mailed thorough FedEx that contained the protected health information (PHI) of 795 individuals was lost in transit. The PHI involved included names, addresses, Social Security numbers, dates of birth, and financial and clinical information. The CE notified HHS, affected individuals, and the media.
