The covered entity (CE), Priority Health, reported that an employee of its business associate (BA) was the subject of an email phishing scheme that compromised the protected health information (PHI) of 36,812 individuals. The PHI involved included names, dates of birth, and Social Security numbers. The CE notified HHS, affected individuals, the media, and revised its breach notification procedure. OCR provided technical assistance regarding the HIPAA Privacy and Breach Notification Rules.
