The covered entity (CE), Peach State Health Plan, reported that its business associate (BA) experienced a cyber-attack that affected the protected health information (PHI) of 1,246 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and the CE and BA implemented additional administrative, technical, and security safeguards to better protect PHI.
