The business associate (BA), OrthoAtlanta LLC, reported that an employee was the target of an email phishing scheme that compromised the protected health information (PHI) of 626 individuals. The PHI involved in the breach included demographic and treatment information. The BA notified the affected covered entities and HHS of the breach. In response to the breach, the BA implemented additional administrative, technical, and security safeguards including additional privacy and security training.
