The covered entity (CE), Orlando Health reported that an employee was the target of an email phishing attack that affected the protected health information (PHI) of 35,403 individuals. The PHI involved includes clinical and demographic information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals, and implemented additional administrative, technical, and security safeguards to better protect its PHI.
