The covered entity (CE), Orlando Health, discovered during audit on May 27, 2015, that an employee was accessing protected health information (PHI) outside the scope of her employment. The PHI contained the names, dates of birth and clinical records of 3,421 individuals. The CE provided breach notification to HHS, affected individuals, and the media and posted substitute notice. In response to the breach, the CE retraining employees. In addition, the CE offered credit monitoring to the affected individuals.
