The business associate (BA), JDA eHealth Systems, reported that an employee mailed the protected health information (PHI) of 917 individuals to the wrong recipients. The PHI involved included names and treatment information. The BA, working with the CE, notified HHS, affected individuals, and the media. In its mitigation efforts, the BA implemented additional administrative, technical, and security safeguards to better protect PHI.
