Health Care Service Corporation, the covered entity (CE), reported that a vendor of its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 543 individuals. The PHI involved included names, birthdates, diagnoses, claims information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE and BA implemented additional administrative, technical, and security safeguards to better protect sensitive data. All staff were retrained.
