Fort HealthCare, Inc., the covered entity (CE), reported that its business associate (BA) inadvertently posted the electronic protected health information (ePHI) of 784 individuals to the Internet. The ePHI involved included names, addresses, telephone numbers, email addresses, dates of birth, Social Security numbers, financial and health insurance information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
