Fort HeathCare, Inc., the covered entity (CE), reported that an employee of its business associate (BA), OS, Inc., was the victim of an email phishing scheme affecting 19,000 individuals. The protected health information (PHI) involved included names, addresses, and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided complimentary identity theft monitoring services.
