Familylinks, the covered entity (CE), reported that an employee was the subject of an email phishing scheme that the protected health information (PHI) of 3,775 individuals. The PHI involved included names, dates of birth, addresses, some Social Security numbers, claims and financial information, and other clinical information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE enhanced the organization’s multifactor authentication requirements.
