The covered entity (CE), Familylinks, reported that its business associate (BA) was the victim of a ransomware attack affecting the protected health information (PHI) of 1,494 individuals. The PHI involved included names, dates of birth, addresses, diagnoses, lab results and medications prescribed. The CE notified HHS, affected individuals, the media, and provided substitute notice. As a consequence, the CE severed its business relationship with the BA.
