The covered entity (CE), the Colorado Department of Human Services, reported that its business associate experienced a cyber-attack that compromised the protected health information (PHI) of 6,132 individuals. The PHI involved included names, addresses, telephone numbers, email addresses, dates of birth, and health insurance information. The CE notified HHS, affected individuals, the media, and provided substitute notice. OCR provided technical assistance to the CE regarding its obligations under the HIPAA Breach Notification Rule.
