The covered entity (CE), Colorado Department of Human Services, mailed the protected health information (PHI) of 12,230 individuals to the wrong recipients. The PHI involved included names, employer information, and financial information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional technical and security safeguards to better protect its PHI.
