The covered entity (CE), Centerstone of Tennessee, reported that an employee was the subject of an email phishing attack that compromised the protected health information (PHI) of 3,675 individuals. The PHI involved included names, dates of birth, drivers’ license and Social Security numbers, addresses, claims and financial information, The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical security measures.
