The covered entity (CE), Centerstone of Tennessee, reported that several employees were victims of an email phishing attack that affected the electronic protected health information (ePHI) of 50,965 individuals. The ePHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, diagnoses/conditions, lab results, medications prescribed, and other treatment information. In response to the breach the CE implemented additional technical safeguards and retrained its employees on email security. OCR provided technical assistance to the CE regarding its Security Rule obligations.
