The covered entity (CE), Yale New Haven Hospital, reported that its business associate experienced a ransomware attack that affected the electronic protected health information (ePHI) of 15, 904 individuals. The ePHI involved included names, addresses, dates of birth, and financial information. The CE notified HHS, affected individuals, and the media. The CE also provided complimentary credit monitoring services.
