The covered entity (CE), Yale New Haven Health Services Corporation, reported that its business associate (BA) was the victim of a cyber-attack that compromised the protected health information (PHI) of 14,603 individuals. The PHI involved included names, addresses, phone numbers, email addresses, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the BA implemented additional technical safeguards to better protect PHI.
