Worthington Steel Health and Welfare Plan

The covered entity (CE), Worthington Steel Health and Welfare Plan, reported that it sustained a ransomware attack that impacted the protected health information (PHI) of 4,078 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, health insurance information, claims and financial information, diagnoses and conditions, and medications. The CE notified HHS, the affected individuals, and the media about the breach. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
