Work Health Solutions

The covered entity (CE), Work Health Solutions, reported that an employee was the victim of an email phishing scheme that compromised the protected health information (PHI) of 13,157 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE secured all its employee email accounts, implemented additional technical safeguards, revised its policies and procedures, and retrained its workforce members. During the investigation OCR provided the CE with technical assistance regarding the HIPAA Rules.
