Wisconsin Department of Health Services, the covered entity (CE), reported that an employee emailed the protected health information (PHI) of 12,358 individuals to other employees and the data was also posted to the Internet. The PHI involved included names, dates of birth, Social Security numbers, and gender. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts the CE provided complimentary identity protection services to affected individuals.
