Wisconsin Department of Corrections, the covered entity (CE), reported that the protected health information (PHI) of 1,723 individuals was viewable over the Internet. The PHI involved clinical and diagnostic information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE revised its policies and procedure regarding the HIPAA Privacy and Security Rules and trained or retrained its workforce. OCR provided technical assistance regarding the HIPAA Rules.
