White Bird Clinic, the covered entity (CE), reported that an employee emailed the protected health information (PHI) of 584 individuals to the wrong recipient. The PHI involved included names, dates of birth, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE revised its policies and procedures and provided complimentary credit monitoring services. OCR provided technical assistance regarding the HIPAA Rules.
