West Holt Memorial Hospital

The covered entity (CE), West Holt Memorial Hospital, reported that an employee was the victim of an email phishing scheme that compromised the protected health information (PHI) of 541 individuals. The PHI involved included names, addresses, birthdates, Social Security numbers, diagnoses/conditions, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its PHI.
