The business associate (BA), WellBe Senior Medical, reported that its vendor experienced a cyber-attack that compromised the protected health information (PHI) of 42,020 individuals. The PHI involved included names, address, dates of birth, lab results, medications, and other treatment information. The BA notified HHS, affected individuals, and the media. In its mitigation efforts, the BA and its vendor implemented additional administrative, technical, and security safeguards to better protect PHI.
