The covered entity (CE), Welfare, Pension and Annuity Funds of Local No. ONE, I.A.T.S.E., reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 20,549 individuals. The PHI involved included names, dates of birth, Social Security numbers, government identification numbers, financial information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards and retrained its employees on cybersecurity awareness practices.
