The covered entity (CE), Weill Cornell Medicine, reported that several employees were the victims of an email phishing scheme that affected the protected health information (PHI) of 25,697 individuals. The PHI involved included names, dates of birth, postal addresses, email addresses, medical record numbers, health insurance information, diagnoses, medications prescribed, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on the proper methods of identifying fraudulent email communications.
