Virginia Gay Hospital

The covered entity (CE), Virginia Gay Hospital, reported that several employees were the victims of an email phishing scheme that affected the protected health information (PHI) of 5,030 individuals. The PHI involved included names, dates of birth, Social Security numbers, claims and financial information, and diagnoses. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative and technical safeguards to better protect its PHI. In addition, the CE retrained its workforce members on the proper methods of identifying fraudulent email communications.
