The Virginia Department of Medical Assistance Services, the covered entity (CE), reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 1,229,333 individuals. The PHI involved included names, birthdates, Social Security and drivers’ license numbers, claims and financial information, and other treatment information. The CE notified HHS and the media. In its mitigation efforts, the CE and BA implemented additional administrative and technical safeguards to better protect PHI.
