Villa St. Francis Catholic Care Center

The covered entity (CE), Villa St. Francis Catholic Care Center, reported that its business associate (BA) experienced a cyber-attack on its network that contained the protected health information (PHI) of approximately 2,522 individuals. The PHI involved names, dates of birth, social security numbers, diagnoses/conditions, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE enhanced its access controls and provided security awareness training to its workforce members. OCR provided technical assistance to the CE regarding the HIPAA Rules.
