The covered entity (CE), Victoria Eye Center/Victoria Surgery Center/Victoria Vision Center, reported that it experienced a hacking attack that compromised the protected health information (PHI) of 80,000 individuals. The PHI involved included clinical information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals, and implemented additional administrative, technical, and security safeguards to better protect its PHI.
