Veterans’ Health Administration, the covered entity (CE), reported that an employee of its business associate (BA) mailed the protected health information (PHI) of 1,847 individuals to the wrong recipients. The PHI involved included names, addresses, and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the BA implemented additional administrative safeguards to better protect and secure PHI.
