Vann Virginia Center for Orthopaedics

Vann Virginia Center for Orthopaedics dba Atlantic Orthopaedic Specialists, the covered entity (CE), reported that an employee was the target of an email phishing scheme that compromised the protected health information (PHI) of 15,264 individuals. The PHI involved in the incident included names, addresses, dates of birth, drivers’ license and social security numbers, claims information, diagnoses/conditions, lab results, and medications. The CE notified HHS, affected individuals, and the media. Substitute notice was provided on the CE’s website. In response to the breach, the CE implemented additional administrative and technical safeguards. Staff were retrained on email security.
