Valley Professionals Community Health Center

The covered entity (CE), Valley Professionals Community Health Center, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 12,029 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, claims and financial information, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. A call center was established for questions or concerns. In its mitigation efforts, the CE strengthened its administrative, technical, and security safeguards to better protect its ePHI.
