The covered entity (CE), UW Medicine, reported that a configuration error in its system resulted in the protected health information (PHI) of 973,024 individuals becoming accessible on the internet. The PHI involved included names and medical record numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice. In response to the breach, the CE implemented additional administrative and technical safeguards and retrained workforce members on methods to better protect its PHI.
