UW Health, the covered entity (CE), reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 2,071 individuals. The ePHI involved included names, addresses, birthdates, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional technical safeguards and retrained its staff in an effort to better protect its sensitive data. OCR obtained assurances that the CE implemented the corrective actions noted.
