UPMC Susquehanna

An unauthorized individual potentially gained access to the email accounts of 11 staff members after they responded to a phishing email. The breach involved the protected health information (PHI) of 1,208 individuals, including names, dates of birth, social security numbers, addresses, and clinical information. The covered entity (CE) provided breach notification to HHS, affected individuals, and the media. Following the breach, the CE implemented stricter password policies and two-factor authentication for its email system. OCR reviewed the CE's policies and procedures to ensure compliance with the Privacy and Security Rules.
