A business associate (BA) employee disclosed the protected health information (PHI) of approximately 2,259 of the covered entity’s (CE) patients to outside parties. The PHI involved in the breach included names, dates of birth, and social security numbers. Following the breach, the CE terminated its relationship with the BA. OCR reviewed the CE’s risk analysis to ensure compliance with the Security Rule.
