University Pediatric Dentistry

The covered entity (CE), University Pediatric Dentistry, reported that multiple employees were the victims of an email phishing attack affecting the protected health information (PHI) of 6,843 individuals. The PHI involved included names, Social Security numbers, drivers’ license numbers, dates of birth, diagnoses, medications prescribed, health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. A call center was established for questions or concerns. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect its PHI.
