Forefront Dermatology dba Premier Dermatology, the covered entity (CE), reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 2,297 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses/conditions, health insurance information, and other treatment information. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect sensitive data.
