The covered entity (CE), University of Maryland Faculty Physicians/University Maryland Medical Center, reported that an employee was the victim of an email phishing scheme that compromised the protected health information (PHI) of approximately 33,896 individuals. The PHI involved included names, dates of birth, Social Security numbers, medical record numbers, and clinical information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
