The covered entity (CE), University of Kentucky HealthCare, reported that its business associate (BA) experienced a ransomware attack affecting the electronic protected health information (ePHI) of 162,589 individuals. The ePHI involved included names, addresses, dates of birth, and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website.
