University of California San Francisco , Privacy Manager Breach

On or about January 11, 2014, unencrypted desktop computers and unencrypted portable computer drives were stolen from the covered entity (CE), University of California San Francisco Family Medicine Center. The types of protected health information (PHI) involved in the breach included names, dates of birth, mailing addresses, medical record numbers, social security numbers, and health insurance identification numbers, affecting 9,861 individuals. The CE provided breach notification to HHS, affected individuals, and the media. In response to the breach, the CE improved physical safeguards, changed or disabled usernames and passwords for accounts that were potentially at risk of compromise, and encrypted the remaining computers at the affected location as well as the replacement computers. OCR obtained assurances that the CE implemented the corrective actions noted above.
