University Medical Center of Southern Nevada, the covered entity (CE), reported that a workforce member impermissibly accessed the protected health information (PHI) of 1,861 individuals. The PHI involved included names, addresses, dates of birth, financial information, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE imposed sanctions on the workforce member and implemented additional administrative and technical safeguards to protect against unauthorized access.
