The covered entity (CE), University Health, reported that an employee of its business associate (BA) uploaded the electronic protected health information (ePHI) of 2,704 individuals to the Internet. The ePHI involved included names, dates of birth, Social Security numbers, addresses, diagnoses, conditions, and other treatment information. The CE notified HHS, the affected individuals, and the media.
