The covered entity (CE), UnitedHealthcare Insurance Company, reported that an employee of its business associate (BA) mailed the protected health information (PHI) of 16,665 individuals to the wrong recipients. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE and BA implemented additional administrative, technical, and security safeguards.
