The covered entity (CE), UnitedHealthcare reported that an employee mailed the protected health information (PHI) of 3,215 individuals to the wrong recipients. The PHI involved demographic and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative safeguards.
